Dear Patients,

This week we will say Farewell to Dr. Priya Moorthy. Dr Moorthy has been working at the Practice for the last 3 years. We wish her all the best for the future.

In the meantime, we will be welcoming a new temporary GP Registrar, Dr Emma Mi. 

Dr Moorthy's  previously registered patients have been now transferred to Dr Mi and the rest of our GPs for continuity of care.

How to register

If you live in our practice area and would like to register with us, please contact reception. You will be asked to complete a registration form and a questionnaire and we require a photographic ID (passport, driving license) if you are joining the Online Access for patients and a proof of address (Official document with your current address on, eg. a utility bill or tenancy agreement).


Alternatively, please print the relevant form from the selection below. The receptionist will check your registration form and take copies of your ID, but will not take your registration if all required information is not provided.

Blood pressure readings and height and weight measurements required for the registration can be made at the surgery. 

When you register with us you will be registering with the practice, however you may state which GP you wish to see.

Please note that the registration process takes up to 72 hours to complete, we can not book an appointment until your registration is complete, if you have moved into the area recently please ensure you register with us as soon as possible.
