Building Work at Botley Medical Centre

Building work will commence on Monday the 8th July at Botley Medical Centre. We apologise for any inconvenience that this may cause advance. We are working towards making the Practice lovely and new for our patients!

FRIDAY DROP IN CLINICS

After trialling and auditing our appointment system, we have found that the number of appointments used during the drop in clinic are actually less than the ones that are offered during a normal clinic. Therefore the Practice have decided to stop this service.

We will be now offering our normal clinics on a Friday which can either be pre booked in advance or booked on the day, to allow more patients to be seen by the Doctors.

  This week Dr Maddy Podichetty has left Botley Medical Centre.

We all wish her the best of luck with her new commitments!

If you were one of her patients you will have been now transferred to a different doctor's list. It can take a bit of time to get used to a new doctor but if you feel you would like to change , please, do not hesitate to let us know in writing and we will do our best to accommodate you with a different doctor.

How to register

If you live in our practice area and would like to register with us, please contact reception. You will be asked to complete a registration form and a questionnaire and we require a photographic ID (passport, driving license) and a proof of address (Official document with your current address on, eg. a utility bill or tenancy agreement).


Alternatively, please print the relevant form from the selection below. The receptionist will check your registration form and take copies of your ID, but will not take your registration if all required information is not provided.

Blood pressure readings and height and weight measurements required for the registration can be made at the surgery. In order to find your NHS number and previous named doctor you may need to contact your previous doctors surgery.

When you register with us you will be registering with the practice, however you may state which GP you wish to see.

Please note that the registration process takes up to 48 hours to complete, we can not book an appointment until your registration is complete, if you have moved into the area recently please ensure you register with us as soon as possible.
